Student Registration

Parents/guardians may register new students at Potomac MS by appointment anytime between the hours of 8:00 a.m. – 3:00 p.m. Monday – Friday.  Walk-ins may experience delays due to the high volume of January registration. Please make an appointment with Stephanie Blackshear (Counseling Secretary/Registrar) at 703-221-4996.

Please bring the following information to register a child for school:

  • Immunization Record
  • Birth Certificate or Notarized Affidavit
  • Social Security card for the student
  • Transcript or most recent report card
  • Copy of IEP (individualized education plan) for students receiving special education services
  • Discipline Report from most recent school attended
  • Proof of legal guardianship (if guardian is not natural parent)
  • Proof of Residency (lease agreement or purchase contract is acceptable)
    • If you are residing with another family you must provide a notarized statement

(Affidavit of Residency) from the property owner indicating that you and the enrolling student reside at that address.

    • If you cannot produce a lease agreement or purchase contract, you must provide two documents to verify your address (example: utility bill, driver’s license, or voter registration card).

 

Student Withdrawal Procedures

Only the legal parent/guardian can withdraw a student from school

 

  1. Please obtain an INTENT TO WITHDRAW form from the guidance department.

    This form can be sent home by your student, faxed to you, or downloaded from this website.

  2. Completely fill out the INTENT TO WITHDRAW form and return it to school at least one day prior to the student’s last day of school.
  3. On the student’s last day of school he/she will report to the guidance department upon arriving to school to pick up the withdrawal forms from the guidance secretary.

    The student will go to each class, return books, clear lockers, and have the teachers sign that items have been returned and include a grade to date.  Students must also clear accounts in the library, cafeteria, and with the school bookkeeper.

    At 2:30 p.m. the student will return to guidance and receive a copy of the

    Completed withdrawal form, class schedule, and report card to take to the receiving school.
